Admissions Information Note: Original forms are required (fax copies will not be accepted). A completed application form and recent photo. Three personal references (one must be your pastor). A non-refundable application fee of $30. Completed medical information and immunization forms. Official transcripts sent directly to the admissions office of all secondary schools and post-secondary institutions attended. A completed church recommendation form. Spouse's personal statement (if applicable). Standardized exam scores (SAT or ACT). These scores are used for placement in english and math courses. Contact the admissions office for information on how to receive these forms.
